Intake

General
Soon, the daycare will start for your son and/or daughter. In order to be as well prepared as possible, we ask you to fill in the intake form as completely as possible. During the intake interview (approximately 1 month before the daycare starts), this form will be discussed with you and there is sufficient opportunity to change things if necessary.

This intake form is part of -what we call- the individual child file. We treat the shared information completely confidentially in accordance with privacy legislation.

As part of the prevention of cot death, our employees turn babies onto their backs during sleep.
Excursion statement(Required)
My child may join me on child-friendly outings outside Villa Yip. Such as walking in the neighborhood, playing in playgrounds, visiting the market (city center) etc.
Image material statement(Required)
Image material is used for various (social) media expressions and/or educational purposes. We only make image material of appropriate situations. For example, we do not photograph during changing moments or in the bedrooms.
Declaration of transfer of primary school(Required)
It is customary that when the child goes to primary school, a (verbal) transfer of data can take place to the future teacher. We advise positively in this, but it is not mandatory. You as a parent decide and make the choice in this.
